- Social and educational programs for families and residents
- Scheduled resident and family meetings
- Trained staff 24 hours a day
- Monthly wellness visits with care plan evaluations by a licensed nurse
- Weekly housekeeping and daily trash removal
- Maintenance of building and grounds
Personalized Services and Assistance
The care plan for each resident is individually determined in consultation with the resident, the family, physician and the staff to insure that all needs and preferences are addressed and personalized. Care plan meetings are conducted at the time of move-in and on a continuous basis and include:
- Personal hygiene/grooming
- Bathing/showering
- Dressing and clothing selection
- Socialization and activities
- Personal Laundry service
- Medication coordination
- Medication management
- Limited medication administration
- Mobility and transferring
- Continence management
- Safety concerns
- Eating ability
- Additional laundry services
This care plan assessment is used to determine the appropriate level of care and service package to insure that dignity and self worth of every resident is maintained and independence is optimized.
Medication Administration
We offer medication administration plans for an additional charge:
- Limited Medication Administration (LMA) offered 8:00 AM – 8:00 PM daily in accordance with all applicable State regulations.
Continence Management
We recognize the importance of continence management in maintaining individual dignity and self-esteem. Our Legacy team, together with the family, physician and resident will tailor a plan that best meets the needs of each resident. We will provide ongoing assessment, including product recommendations and continence plans.
